Services Provided: General counseling: • In-home counseling for people who are not mobile. Home Safety: • Provide home and safety inspections, i.e. assessing the need for grab bars and identifying falling hazards in the home. Then recommending and connecting patients to the resources for these needs. Resource Connections: • Connect people to local recourses such as Meals on Wheels, transportation, caregivers, etc. Emotional preparation: • Emotionally and practically prepare patients and their family members prior to Hospice coming in to the home. With compassionate counseling I can help everyone deal with the impending changes and potential complications. Loss Counseling: • Counsel and encourage people who have experienced a loss in their lives to set new goals, giving them more possibilities to look forward to. |
